Healthcare providers are becoming more aware of the impact that community characteristics have on the patient’s health. Some call it “the social determinants of health” or “health inequity.” Regardless, the trend is for these community characteristics to be considered when managing a patient’s health-based condition. Data is becoming increasingly available to support this effort and it can impact provider outcomes. This presentation is aimed at demonstrating one way this can be accomplished.
