The DFW Hospital Council (DFWHC) will be co-hosting a webinar with CarePort Health to detail “Care Transitions and Coordination” this Thursday from 10:00 a.m. To 11:00 a.m. CDT.

With many hospitalized COVID-19 patients requiring post-acute care to continue their recovery, managing patient transitions can be challenging. To help its acute and post-acute partners during the COVID-19 pandemic, CarePort implemented COVID-19 product features to streamline the hospital discharge process and foster safe patient transitions from the hospital to the appropriate post-acute setting–which in turn helps protect staff and patients and preserve valuable personal protective equipment.
