SUMMARY:DFWHC Foundation's "REDUCING 30-DAY Readmissions"
DESCRIPTION:The DFW Hospital Council (DFWHC) Foundation will host the two-part webinar “REDUCING 30-DAY Readmissions by Improving Care Transitions” on Thursday\, March 2 and Tuesday\, April 18. The event will take place from 1:00-2:00 p.m.\, CT on both days.\n\nJackie Conrad\, MBA\, BS\, RN\, an improvement advisor at Cynosure Health\, will serve as speaker.\n\nObjectives will include:\n\n• A review of your hospital data on All Cause Readmissions\, with data provided by the DFWHC Foundation;\n• Comparing your hospital data to the regional average;\n• Learning how to interview a readmitted patient using the Discovery tool;\n• Learning best practices;\n• Learning from peers and with an open dialogue.\n\nTarget audience includes case managers\, quality experts\, bedside caregivers\, CNOs and anyone interested in reducing readmissions.\n\nYou can register here.\n\nFor information\, contact Patti Taylor at ptaylor@dfwhcfoundation.org.\n\n\n\nIn support of improving patient care\, this activity has been planned and implemented by the University of North Texas Health Science Center at Fort Worth and DFW Hospital Council Foundation.
